65 year old man swerves to avoid a deer, has a head on collision with a tree instead. One week later, his wife finds him confused and unresponsive. He goes to an outside hospital, has his head scanned, siezes and is transferred to our hospital. The neurologist believes he is encephalopathic and wants a lumbar puncture, but he weighs 290 pounds and the LP is unsuccesful after 2 hours of attempting by an upper lever resident AND an attending critical care medicine doctor. So he gets 2 kinds of antibiotics, an antiviral, and is loaded with antisiezure medicine. Today, he woke up and nobody really knows what happened to him.
